重症超声联合血清指标预测重症肺炎转归的价值

摘要:

目的 研究重症超声检查联合血清指标预测重症肺炎(SP)临床转归不良的临床价值。方法 将2022年1月至2024年1月期间四川大学华西医院广安医院收治的162例SP患者按照治疗1周时病情转归情况分为好转组(105例)与恶化组(57例)。采用Pearson相关性和受试者工作特征(ROC)曲线分析血清指标与肺部超声评分(LUS)预测SP临床转归的价值。结果 入院时LUS、血清IL-6、PCT与急性生理与慢性健康(APACHEⅡ)评分呈正相关(r=0.309、0.307、0.288,P<0.05)。入院时LUS、血清IL-6、PCT联合预测的曲线下面积高于LUS、血清IL-6、PCT单独预测(P<0.05)。结论 LUS联合血清IL-6、PCT指标对SP临床转归不良具有一定预测价值。

Abstract:

Objective To study the clinical value of the combination of severe ultrasound examination and serum indicators on the prediction of poor clinical outcomes of severe pneumonia(SP).Methods A total of 162 patients with SP who were admitted to the hospital between January 2022 and January 2024 were categorized into the improvement group(105 cases) and the deterioration group(57 cases) according to the disease outcomes at 1 week of treatment.Pearson correlation analysis and receiver operating characteristic(ROC) curve were utilized to analyze the value of serum indicators and lung ultrasound score(LUS) on predicting the clinical outcomes of SP.Results Pearson correlation analysis showed that LUS,serum interleukin-6(IL-6) and procalcitonin(PCT) at admission were positively correlated with Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ) score(r=0.309,0.307,0.288,P<0.05).The area under the curve of combined prediction with LUS,serum IL-6 and PCT was higher than that of LUS,serum IL-6 or PCT(Z=2.113,2.649,2.047,P=0.035,0.008,0.041).Conclusion LUS combined with serum IL-6 and PCT demonstrates certain predictive value on the poor clinical outcomes of SP.
